This national Sweepstakes is sponsored by ETL Promotions, LLC (ETL Promotions, LLC, P.O. Box 6921, Lee’s Summit, MO 64064-9998), and its independent distributors, and in some locations by independent, unaffiliated co-sponsors. By completing the Official Entry Form, you agree that the sponsors and co-sponsors of this Sweepstakes may call you, even if your number is found on the Do Not Call Registry. You may also be required to confirm information you have provided on the Official Entry Form. Furthermore, you understand that completing the Official Entry Form may result in telephone solicitations. No oral representations or promises are binding on sponsors or co-sponsors.

1. To enter, complete an Official Entry Form available at various locations around the USA (no photocopies) and deposit it in an available collection box, or give the Official Entry Form to your local ETL Promotions, LLC distributor or Sweepstakes co-sponsor, or mail the Official Entry Form to ETL Promotions, LLC, P.O. Box 6921, Lee’ Summit, MO 64064-9998. You may obtain an Official Entry Form by mailing a self-addressed stamped envelope along with a request for an Official Entry Form to ETL Promotions, LLC. ETL Promotions, LLC is not responsible for lost, delayed or misdirected mail.
2. All entries must be received no later than December 31 of the current year. Only one entry per household is permitted. Grand Prize Winner will be determined by random drawing on January 15, of the following year and notifi ed within 2 business days. Odds of winning depend on number of entries received. Entries become the property of ETL Promotions, LLC Judges’—decisions are final in all respects.
3. Grand Prize displayed is not the actual Grand Prize that will be given away. Grand Prize displayed is the sole responsibility of the independent distributor or independent, unaffiliated co-sponsors and must not exceed a maximum value of $25,000. Grand Prize Winner will receive choice between displayed prize or $25,000 cash paid in a lump sum from ETL Promotions, LLC.
4. This sweepstakes is open to all legal United States residents 18 and over and legal residents of Canada (excluding Quebec) who have reached the age of majority in their province or territory of residence and are homeowners at the time of entry. Employees of the sponsors and co-sponsors, and their affiliates, subsidiaries, representatives, agents, dealers and the families of each are not eligible to participate. This Sweepstakes is void in Quebec and where prohibited by law.
5. Sweepstakes Grand Prize Winner must sign a Release of Liability and Photography Release agreeing to the use of his or her name and likeness for publicity and promotional purposes without additional compensation. The Grand Prize Winner and all entrants release the sponsors and co-sponsors from any liability arising from or relating to this Sweepstakes. This Sweepstakes is void where prohibited by law. No transfer or substitution of prizes is allowed. Federal, state and other taxes or charges are the sole responsibility of the Grand Prize Winner.
